Voluntary Membership sa SSS at Philhealth
Simula nung nagwork-at-home ako, natigil na ang contributions ko sa SSS, Pag-ibig at Philhealth. Since gusto kong ipagpatuloy ang paghulog ko, nagpasya akong mag-voluntary member.
SSS
Dati akong employed at ngayon ay nagtra-trabaho at home o self-employed (kasi wala akong employer sa Pilipinas). So ganito ang ginawa ko. Pumunta ako sa SSS Makati, at humingi ng form for voluntary. Pwede ring magdownload sa website ng SSS at magbayad sa bangko. Kung kukuha ka ng form, bibigyan ka ng SSS ng 3 copies ng Form RS-5. Kung gusto mo naman magdownload na lang ng form RS-5, copy and paste this sa browser mo:

Medyo malaki ang pumatak sa contribution for me. Yung totoo, ang dapat na hulog ko ay P1560 a month, kaya lang medyo nalalakihan ako, kaya ginawa ko na lang P520/month.
Siguraduhin mong na-fill up-an lahat at napirmahan mo. Then magprint ka ng 3 copies. Tapos punta ka na sa bangko. Ako, mas pinili ko magbayad sa BPI, pero pwede rin sa ibang bangko. Pwede rin sa BDO.
Pwede kang magbayad quarterly, semi-annually, at annually.
Then, uwi ka na, itabi mo na ang form. Kailangang itabi mo ang form para maging reference at katunayan na naghulog ka.
PHILHEALTH
Sa Philhealth, hindi pwedeng basta na lang kumuha ng forms for voluntary at magbayad sa bangko. Kailangang pumili sa Philhealth Makati, sa Buendia rin para mag-update ng status. Nag-update ako ng status from Employed to Individually Paying. Kumuha ako ng form sa guard par mag-update ng status, finill-upan ko ang form, then pumila na ako. Mabilis naman natapos ang pila, about 15 minutes, kaya okay lang. Pagka-update ng record ko sa computer, binigyan ako nung nag-assist ng Philhealth Record ko. Sabi niya pwede akong magbayad sa bangko, pero para madali, dun na rin daw ako mismo magbayad.
Iba pa ang pila kapag magbabayad, pero yung nag-assist sa akin sa pag-update ng form, ibinigay na lang niya sa cashier nila yung form ko para di nako pumili. Nagbayad ako ng P300 para sa quarterly contribution. That was for the months of October, November and December 2010. Binigyan ako ng receipt.
So meron na akong Philhealth record, and receipt para dun sa binayaran ko. Of course, itinabi ko rin yun.
Ang susunod na bayad ko ay dapat hindi lalampas sa March 30, 2011 deadline.
Sa susunod, annually na ibabayad ko, so that's P1200 lang, para isang beses isang taon na lang ako pipila.
Gusto ko sanang kumuha ng Philhealth ID, kaya lang di ito available sa Makati Branch. Kailangan daw pumunta pa ako sa Pasig branch, sa main.
